Shamsul Islam, PharmD, MBA

Dr. Islam is the Director of Pharmacy at Mather Hospital. He received his Doctor of Pharmacy degree from Long Island University’s Arnold and Marie Schwartz College of Pharmacy and Health Sciences in Brooklyn, NY. He then completed his Master of Business Administration degree with a focus on Healthcare Management from Hofstra University’s Frank G. Zarb School of Business in Hempstead, NY. Dr. Islam serves as the Co-Chair for the Pharmacy and Therapeutics Committee and an active member of many Northwell system committees that are responsible for optimizing pharmacy-related care systemwide. Dr. Islam’s areas of interest include leadership, administration and the recently completed Northwell’s LeadNEXT program. In addition, he is involved in initiatives related to patient & medication safety, research, and teaching and mentoring students and resident pharmacists. 

Maricelle O. Monteagudo-Chu, PharmD, BCIDP, BCPS

Dr. Monteagudo-Chu is a Senior Clinical Pharmacy Specialist and is currently the PGY-1 Pharmacy Residency Program Director. She received her Doctor of Pharmacy degree from Rutgers University Ernest Mario School of Pharmacy in 2007. She completed her PGY1 Pharmacy Residency at Kingsbrook Jewish Medical Center in 2008 and PGY2 Infectious Diseases Pharmacy Residency at James J. Peters VA Medical Center in 2009. Dr. Monteagudo-Chu is a Board-Certified Infectious Diseases Pharmacist (BCIDP) and Board-Certified Pharmacotherapy Specialist (BCPS). 

Dr. Monteagudo-Chu is the Co-Chair of the Antimicrobial Stewardship Program (ASP) at Mather Hospital. She is involved in antimicrobial stewardship initiatives, guideline development and education throughout the hospital and Northwell Health System ASP. Dr. Monteagudo-Chu is an active member of American College of Clinical Pharmacy (ACCP), American Society of Health-System Pharmacists (ASHP) and Society of Infectious Diseases Pharmacists (SIDP). She is a member of the SIDP International Task Force and served as the SIDP Stewardship Committee Chair (2022-2023) and Past-Chair (2023-2024). Dr. Monteagudo-Chu also enjoys spending her leisure time traveling and unwinding with her family and dog Murphy.  

 

Francis Angilly, RPh, MBA

Mr. Angilly earned his Bachelor of Science in Pharmacy degree from St. John’s University in 2001 and recently completed his Master of Business Administration in Healthcare Management from St. Joseph’s University in 2024. Mr. Angilly began his career in retail pharmacy, where he served as the Pharmacist-in-Charge for multiple stores. In 2015, he transitioned to the hospital setting, joining Mather Hospital as a per-diem pharmacist. He quickly progressed to the role of Hospital Pharmacist on the evening shift and was subsequently promoted to Pharmacy Supervisor. As Pharmacy Supervisor, Mr. Angilly is responsible for the daily operations of the pharmacy department. He collaborates closely with Shamsul and Amanda to ensure efficient and safe medication dispensing while maintaining compliance with regulatory standards. Additionally, he oversees the procurement of medications for the department. When not at work, Mr. Angilly enjoys spending time with his family and watching movies. 

Sarika Patel-Adib, PharmD  

Dr. Patel-Adib received her Doctor of Pharmacy degree from Northeastern University in 2015. She began her career in the retail pharmacy setting and transitioned to the hospital setting in 2019. Dr. Patel-Adib currently oversees all activities related to sterile compounding. She manages the compounding personnel and ensures compliance with USP 797/800. She also assists with drug diversion investigations and ongoing pharmacy projects.  In her free time, Dr. Patel-Adib enjoys spending time with her family, traveling and trying new restaurants.  

Nilupar Rahman, PharmDBCPS

Dr. Rahman completed her Doctor of Pharmacy degree from St. John’s University in 2003. Her pharmacy career began in the retail/community setting and then transitioned as a hospital pharmacist.  As a hospital pharmacist, her experiences include working in the Emergency Department and rounding with the Internal Medicine team. Dr. Rahman is currently the Transitional Care Unit pharmacist, in which she optimizes medication management and provides patient education.  Dr. Rahman is a Board-Certified Pharmacotherapy Specialist.

Dr. Rahman is also actively participating in an ongoing collaborative quality improvement project in the Telemetry unit to enhance patient medication education. She also works closely with the Diabetes Educator to provide continuing education to nurses regarding diabetes pharmacotherapy. Dr. Rahman’s other activities include providing data for the annual Leapfrog survey and being a proud member of the Equity, Diversity, Inclusion and Belonging Committee. Dr. Rahman hobbies include exercising, reading, and watching Rom-Coms from the 80’s and the 90’s.

Martin Sexton, PharmD, BCNSP 

Dr. Sexton received his Bachelor of Science in Pharmacy degree from St. John’s University then later pursued his Doctor of Pharmacy degree from University of Arkansas for Medical Sciences (UAMS) in 2008. Dr. Sexton has been a practicing pharmacy in the hospital setting for over 30 years. He attends daily multidisciplinary rounds in the critical care and internal medicine units, ensuring optimal patient care. He is also a member of the Nutrition Support team at Mather Hospital working with Registered Dietitians and implementing initiatives involving nutrition support for our patients. Dr. Sexton is a Board-Certified Nutrition Support Pharmacist (BCNSP) and an active member of the American Society of Parenteral and Enteral Nutrition (ASPEN).   

Amanda Tommasiello, PharmD, BCPS, PRS

Dr. Tommasiello earned her Doctor of Pharmacy degree with a minor in Psychology from St. John’s University in Jamaica, NY, in 2011. She is a Board-Certified Pharmacotherapy Specialist and is currently pursuing an MBA in Healthcare Strategic Management at Hofstra University’s Frank G Zarb School of Business in Hempstead, NY. In her current role as Senior Manager, she supports the Director in managing the daily operations of the Inpatient Pharmacy and Employee Prescription department at Mather Hospital. Dr. Tommasiello is instrumental in maintaining adherence to legal and regulatory standards, actively engaging in hospital committees such as Pharmacy & Therapeutics (P&T) and the Performance Improvement & Compliance Group (PICG). She leads drug diversion investigations, adverse drug event reporting, and medication error analysis. With a strong focus on medication safety, pharmacy informatics, and regulatory compliance, Dr. Tommasiello is committed to enhancing pharmacy practice and patient care through innovative solutions and stringent safety protocols. In her downtime, Dr. Tommasiello enjoys spending time with family and finds crafting to be a calming outlet for her creativity. 

Tyler Yarema, PharmD 

Dr. Yarema earned his Doctor of Pharmacy degree from Duquesne University in 2016. He began his career as the Pharmacist-in-Charge at CVS Pharmacy, then transitioned to the role of Emergency Medicine Pharmacist at Mather Hospital. Dr. Yarema completed the Emergency Medicine Certificate Program through the American Society of Health-System Pharmacists. As an integral member of the Emergency Medicine team, he responds to codes, conducts medication reconciliation, and educates hospital staff and patients about medications. Dr. Yarema actively participates in hospital councils and committees. He recently contributed to the ED’s achievement of GEDA Geriatric Level 1 accreditation and received a DAISY Team Award for exceptional patient care. Outside of the pharmacy, Dr. Yarema enjoys health and wellness research, participating in triathlons, and spending time with his two Bernedoodles.
