PORT JEFFERSON, NY – Mather Hospital was awarded the American Heart Association/American Stroke Association’s 2022 Get With The Guidelines® – Stroke GOLD PLUS with Target: Stroke Honor Roll Elite and Target: Type 2 Diabetes Honor Roll award in recognition of its quality patient care.

The stroke award recognizes the hospital’s commitment to ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines based on the latest scientific evidence. Mather earned the award by meeting specific quality achievement measures for the diagnosis and treatment of stroke patients. Before discharge, patients also receive education on managing their health, get a follow-up visit scheduled, as well as other care transition interventions.

The Honor Roll Elite award is given to hospitals that achieve door-to-needle times within 60 minutes for at least 85 percent of applicable patients being treated with tPA clot-dissolving medicine.

Target: Type 2 Diabetes Honor Roll aims to ensure patients with Type 2 diabetes receive the most up-to-date, evidence-based care when hospitalized with cardio-vascular disease or stroke. To bring attention to this critical high-risk population, American Heart Association has established the Target: Type 2 Diabetes Honor Roll recognition opportunity for hospital participants of Get With The Guidelines®–Heart Failure and Get With The Guidelines®-Stroke.
